Roles & Responsibilities

Provide compassionate bedside care to residents and patients as a licensed practical nurse
Administer medications and treatments accurately and document all care
Monitor patient condition and promptly report changes to the RN or provider
Collaborate with the interdisciplinary team to support individualized care plans
Assist with admissions, transfers, and discharges with professionalism
Maintain accurate, timely documentation in patient records to ensure continuity of care
Support a safe, clean environment and follow facility policies and procedures

Qualifications

Valid LPN license in Pennsylvania (required)
Current BLS/CPR certification (required or obtained upon hire)
Previous experience in long-term care or skilled nursing preferred
Ability to pass facility-required health screenings, background check, and drug screen

Education

Completion of an accredited LPN/licensed practical nurse program
High school diploma or equivalent

Knowledge & Skills

Strong medication administration and documentation skills
Skilled in patient assessment and vital signs monitoring
Experience caring for geriatric and rehabilitation populations is a plus
Clear communication, teamwork, and time management skills
