Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents as a Licensed Practical Nurse.
Administer medications and treatments accurately and in line with facility protocols.
Monitor and document patient conditions and promptly communicate changes to the care team.
Collaborate with RNs, physicians, and support staff to develop and implement individualized care plans.
Educate residents and families about care plans, medications, and safety measures in a clear, kind way.
Maintain accurate, timely clinical documentation and ensure smooth shift handoffs.

Qualifications

Current LPN license in Pennsylvania (required).
CPR/BLS certification or willingness to obtain prior to start.
Prior experience in long-term care, skilled nursing, or a similar setting is preferred.
Strong communication, organization, and patient-care skills — you keep calm and carry a stethoscope.
Ability to pass background checks and required health screenings.

Education

Completion of an accredited LPN/LVN program.
High school diploma or GED.

Knowledge & Skills

Medication administration and precise documentation.
Vital signs assessment and routine patient monitoring.
Basic wound care and assistance with activities of daily living (ADLs).
Familiarity with electronic medical records (EMR) or the ability to learn quickly.
Team collaboration, patient-centered care, and strong interpersonal skills.
