Roles & Responsibilities

• Administer medications and treatments safely and accurately according to care plans
• Monitor and document patient condition, vital signs, and responses to interventions
• Provide compassionate hands-on care and assist with activities of daily living
• Collaborate with RNs, providers, and interdisciplinary team members to support patient outcomes
• Maintain clear, timely documentation in the electronic health record
• Support infection control, safety protocols, and a clean therapeutic environment
• Respond calmly and effectively to changes in patient condition and urgent needs
• Contribute to a positive patient experience by communicating with empathy and respect

Qualifications

• Current Licensed Practical Nurse (LPN) license in the state of practice or eligible for licensure
• Basic Life Support (BLS) / CPR certification required or obtained upon hire
• Comfortable working overnight shifts and adapting to per-diem scheduling
• Professional bedside manner with strong communication and teamwork skills
• Prior experience in long-term care, skilled nursing, or a similar setting preferred

Education

• Graduate of an accredited Practical Nursing program
• High school diploma or equivalent required
• LPN licensure and BLS/CPR certification

Knowledge & Skills

• Proficient in medication administration and documentation
• Skilled in taking and interpreting vital signs and recognizing changes in condition
• Familiarity with wound care and basic nursing procedures
• Comfortable using electronic health records (EHR) for charting
• Strong time management, organization, and prioritization abilities
• Ability to communicate clearly with patients, families, and care teams
