Roles & Responsibilities

• Deliver direct patient care with compassion and clinical accuracy
• Administer medications and treatments according to care plans and facility policies
• Monitor patient status, document changes, and communicate concerns to the care team
• Perform assessments, record vitals, and assist with ADLs to promote comfort and dignity
• Collaborate with nurses, therapists, and physicians to implement individualized care
• Maintain accurate, timely documentation in the charting system
• Support a safe environment by following infection control and safety protocols
• Mentor and assist CNAs and other staff when appropriate to ensure high-quality care

Qualifications

• Current LPN license in good standing (Pennsylvania license required or eligible)
• Basic Life Support (BLS) certification or ability to obtain upon hire
• Current immunizations and ability to pass standard background and health screenings
• Comfortable with medication administration, wound care basics, and routine assessments
• Compassionate communicator who works well on a multidisciplinary team

Education

• Graduate of an accredited Practical Nursing program; LPN diploma or equivalent required

Knowledge & Skills

• Strong clinical assessment and bedside care skills
• Medication administration and documentation proficiency
• Familiarity with long-term/Skilled Nursing care practices
• Good charting and electronic health record (EHR) skills
• Clear communication, time management, and teamwork abilities
• Ability to follow protocols for infection control and patient safety
