Roles & Responsibilities

Provide compassionate direct patient care and bedside support
Administer medications and treatments safely and accurately
Monitor patients’ conditions and report changes to nursing staff
Document assessments, interventions, and patient responses clearly
Collaborate with interdisciplinary team members to support care plans
Support admissions, discharges, and patient education with professionalism

Qualifications

Active LPN (Licensed Practical Nurse) license in Pennsylvania or compact/state-appropriate licensure
Current Basic Life Support (BLS) certification, or willingness to obtain
Clean background check and ability to meet facility onboarding requirements
Strong communication skills and a patient-first attitude
Previous long-term care or skilled nursing experience preferred but not required

Education

LPN certification/license required
High school diploma or GED required

Knowledge & Skills

Medication administration and basic clinical assessments
Familiarity with electronic medical records and accurate charting
Infection control practices and safe patient handling
Prioritization and time management in a busy care setting
Teamwork and effective communication with patients and families
