Roles & Responsibilities

Deliver compassionate, hands-on nursing care to residents and patients
Administer medications and treatments accurately and on schedule
Monitor and document vital signs, patient status, and changes in condition
Collaborate with RNs, CNAs, and the interdisciplinary team to create great care plans
Provide clear, empathetic communication with residents, families, and staff
Maintain timely and accurate charting in the facility record system

Qualifications

Active LPN (Licensed Practical Nurse) license — ready to practice
Completion of an accredited practical nursing or vocational nursing program
Current Basic Life Support (BLS/CPR) certification (or willingness to obtain)
Comfort with medication administration, wound care basics, and patient assessments
Strong communication skills and a patient-centered attitude
Previous LPN experience in long-term care or skilled nursing preferred

Education

Graduate of an accredited LPN/Vocational Nursing program
Current state LPN licensure required

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care and ADL support
Patient assessment and vital sign monitoring
Familiarity with electronic medical records (EMR) and accurate charting
Ability to prioritize tasks, remain calm under pressure, and work as a team
