Roles & Responsibilities

• Verify and clinically review oncology and infusion medication orders to ensure safety and efficacy
• Prepare and oversee sterile compounding in the IV room, following hazardous and non-hazardous drug protocols
• Perform clinical dosing and pharmacokinetic assessments for high-alert oncology medications
• Document clinical interventions and medication reconciliation in Epic to maintain accurate patient records
• Collaborate with oncologists, nurses, and the interdisciplinary care team to optimize patient outcomes
• Provide patient counseling and education on chemotherapy regimens, side-effect management, and safe handling
• Ensure compliance with sterility standards, PATT testing, and packaging requirements for antineoplastic agents
• Participate in quality improvement initiatives and medication safety programs specific to oncology care

Qualifications

• California Pharmacist license (required)
• PharmD (Doctor of Pharmacy) — PharmD noted as required in the role
• Minimum 2 years recent Oncology clinical infusion experience (required)
• Epic experience (required) for charting and order management
• Sterile compounding/IV room experience with hazardous and non-hazardous drugs
• Certifications preferred: BLS, Chemo/Hazardous Drug certification, IV Certification
• RPh designation and experience with medication dispensing systems (Baxter, Cad cassette) preferred
• Must be able to pass PATT testing as required by facility

Education

• PharmD (Doctor of Pharmacy) required

Knowledge & Skills

• Strong clinical dosing and pharmacotherapy management skills for oncology patients
• Expertise in sterile technique, IV preparation, IV piggyback and IV push medication workflows
• Familiarity with packaging antineoplastic agents and handling high-alert medications safely
• Competence in therapeutic drug monitoring and pharmacokinetic dosing
• Experience with inpatient/acute care and compounding pharmacy workflows
• Comfortable using medication dispensing systems (Baxter, Cad cassette) and infusion technologies
• Excellent communication skills for patient education and multidisciplinary collaboration
