Roles & Responsibilities

• Coordinate and manage inpatient care plans to ensure timely, safe discharges
• Perform utilization review and case management to optimize patient placement
• Lead discharge planning and needs assessments, including DME orders
• Document thoroughly in Epic to maintain accurate, compliant charts
• Collaborate with interdisciplinary teams (PT, RT, pharmacy, social work) to streamline care
• Conduct audits and DRG reviews to support regulatory compliance
• Float between units as needed within scope to meet facility staffing demands
• Serve as charge nurse/lead when required and support unit leadership

Qualifications

• Washington RN license or Compact RN license required
• BLS certification required
• Minimum 2 years recent inpatient acute care hospital experience (within last 3 years) — HARD STOP
• Previous charge experience required
• Epic charting experience required
• Demonstrated experience with care coordination, discharge planning and utilization review
• Ability to work EOW (every other weekend) and float between units as needed
• Pending licenses not accepted; local candidates not accepted (50-mile local radius rule applies)

Knowledge & Skills

• Care Management and Utilization Review expertise
• Strong knowledge of admission criteria, discharge planning and care coordination
• Familiarity with DRG, CMS regulations, HIPAA and The Joint Commission standards
• Proficient Epic charting skills
• Comfort working in acute care specialties: ICU, PCU/SDU/IMC/OBS, ED, Women’s Services
• Skilled at performing audits and supporting regulatory documentation
• Ability to manage high patient ratios (experience with 1:30 patient ratio noted)
• Excellent communication and interdisciplinary collaboration skills
