Roles & Responsibilities

• Perform comprehensive evaluations to assess residents’ speech, language, and swallowing needs
• Provide daily treatments and individualized therapy interventions
• Plan and execute safe and effective discharge processes
• Document treatment plans and progress accurately and timely
• Collaborate with the interdisciplinary therapy team to optimize patient outcomes
• Follow facility and company policies and procedures (CRG P & P) to maintain quality standards

Qualifications

• SLP — primary credential/role required (Speech-Language Pathologist)
• 1-3 years of clinical experience (job specifies 1-3 years)
• Prior SNF experience strongly preferred (explicitly requested)
• Ability to work on-site and adhere to facility dress code and policies
• Comfort meeting productivity expectations (therapist productivity requirement: 90%)

Knowledge & Skills

• Skilled in evaluations, daily treatments, and discharges (core expectations)
• Strong documentation and clinical reasoning skills
• Effective teamwork and communication within a multidisciplinary therapy team
• Familiarity with skilled nursing facility workflows and resident-centered care
• Ability to follow organizational policies and meet productivity benchmarks
