Roles & Responsibilities

• Assess patients' mobility, function, and rehabilitation needs in the SNF setting
• Develop and implement individualized treatment plans to restore functional independence
• Document evaluations, progress notes, and discharge summaries per CRG policies
• Collaborate with nursing and interdisciplinary teams to coordinate patient-centered care
• Meet productivity expectations and help the team deliver consistent high-quality therapy (90% productivity requirement)
• Provide patient and family education to support safe transitions and ongoing recovery
• Adhere to facility policies and protocols, including dress code (black scrubs) and CRG P&P

Qualifications

• PT or DPT credentials listed as primary skills (PT, DPT)
• 1-3 years of clinical experience (job specifies 1-3 years)
• Experience or comfort working in a Skilled Nursing Facility (SNF) practice setting
• Ability to be onsite to receive guaranteed hours per facility policy
• Willingness to follow CRG policies and productivity standards; able to meet 90% productivity
• Comfortable with facility dress code (black scrubs) and agency interview process

Education

• DPT (Doctor of Physical Therapy) or PT credential

Knowledge & Skills

• Skilled Nursing Facility therapy practices and SNF documentation
• Patient assessment, therapeutic exercise, and mobility training
• Treatment planning and measurable goal setting
• Interdisciplinary communication and care coordination
• Time management and productivity within a fast-paced clinical environment
• Professionalism, adherence to policies, and reliable onsite presence
