Roles & Responsibilities

• Assess residents' communication and swallowing needs and create patient-centered plans of care
• Provide skilled speech-language therapy interventions in a Skilled Nursing Facility setting
• Collaborate with nursing and interdisciplinary team members to support resident goals and continuity of care
• Document evaluations, progress notes, and discharge plans accurately and timely
• Educate residents, families, and caregivers on strategies to carryover progress into daily life
• Track and meet productivity expectations while maintaining high-quality care
• Adhere to facility and company policies and procedures to ensure safe, compliant practice

Qualifications

• 1-3 years of SLP experience (years of experience listed as 1-3 in the job details)
• Experience or comfort working in a Skilled Nursing Facility (SNF) setting
• Willingness to follow CRG policies & procedures and meet a 90% productivity requirement
• Ability to be present in the building to receive guaranteed hours and to follow facility dress code (black scrubs)
• Flexibility to work a contract travel assignment and comply with contract terms (can cancel contract with 2 weeks notice)

Knowledge & Skills

• Strong assessment and treatment planning skills for adult speech, language, cognition, and swallowing disorders
• Skilled documentation and Medicare/SNF-related paperwork familiarity
• Effective communication and teamwork within an interdisciplinary rehab team
• Time management and productivity-focused practice to meet facility expectations
• Professionalism in a clinical setting and adherence to infection control and safety standards
