Roles & Responsibilities

• Evaluate residents' speech, language, cognitive-communication and swallowing function with thorough, person-centered assessments
• Develop and implement individualized treatment plans to help residents regain function and independence
• Deliver skilled therapeutic interventions using evidence-based techniques appropriate for a SNF population
• Collaborate closely with nursing, therapy teammates, families and care teams to drive positive resident outcomes
• Document progress clearly and timely to meet facility and regulatory requirements
• Meet productivity expectations and contribute to a high-performing therapy program
• Follow Continuum Rehabilitation Group (CRG) policies and facility procedures while maintaining professional standards

Qualifications

• Licensed or eligible for licensure as an SLP in New Mexico (verify state requirements)
• 1-3 years of clinical experience preferred (SNF/geriatric experience a plus)
• Comfortable working in a skilled nursing facility and collaborating with interdisciplinary teams
• Reliable, on-site presence (therapist must be present in the building to receive guaranteed hours)
• Ability to meet productivity expectations and follow employer policies
• Professional appearance consistent with facility dress code (black scrubs)

Knowledge & Skills

• Strong assessment skills for dysphagia, speech, language and cognitive-communication disorders
• Experience with documentation systems and regulatory compliance in a SNF setting
• Effective communication and interpersonal skills for resident- and family-centered care
• Ability to prioritize caseloads and meet productivity/quality targets
• Team-oriented mindset and willingness to collaborate across disciplines
