Roles & Responsibilities

• Conduct comprehensive psychosocial and clinical assessments for patients across the lifespan
• Develop individualized treatment plans using evidence-based approaches
• Deliver therapeutic interventions addressing depression, anxiety, trauma, substance use and adjustment disorders
• Collaborate closely with primary care providers, nurses, and behavioral health colleagues to coordinate whole-person care
• Document clinical encounters and treatment progress thoroughly and professionally
• Engage with patients and families to provide education, resources and linkages to community supports
• Advocate for the needs of underserved and rural populations within the care team
• Participate in case consultations and contribute to continuous quality improvement initiatives

Qualifications

• Active LISW license in Iowa (required)
• Master's degree in Social Work (MSW) from an accredited program (required)
• Minimum of 2 years of clinical social work experience (preferred)
• Experience working in rural or medically underserved settings is a plus
• Strong clinical documentation and organizational skills
• Proven ability to work collaboratively within an integrated care team
• Comfort working with diverse lifespan populations and a range of behavioral health diagnoses

Education

• Master's degree in Social Work (MSW) from an accredited program

Knowledge & Skills

• Skilled in psychosocial assessment and treatment planning
• Experience delivering evidence-based therapeutic interventions (e.g., CBT, trauma-informed care)
• Knowledge of substance use treatment approaches
• Strong interpersonal and communication skills for patient and team interactions
• Familiarity with integrated care models and collaborative workflows
• Competent clinical documentation and case management abilities
