Roles & Responsibilities

• Provide skilled physical therapy care in patients’ homes, delivering patient-centered interventions
• Perform thorough functional assessments and develop individualized treatment plans
• Implement therapeutic exercises, gait training, and mobility interventions to maximize independence
• Document visits accurately and timely in the charting system (Epic preferred)
• Coordinate care with nurses, aides, and other members of the interdisciplinary team
• Educate patients and caregivers on home exercise programs, safety, and fall prevention
• Complete Trinity specialty-specific competency testing and skills checklists as required
• Maintain required health screenings and immunizations to meet agency standards

Qualifications

• Valid Physical Therapist license (Iowa) — license/certificate verification required
• BLS certification (required)
• Completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Current fit test and required health screenings (MMR, Hep B, COVID-19 vaccine, Tdap, TB test or IGRA, Varicella, drug screen)
• Epic experience preferred but not required
• Comfort working independently in home settings and strong patient education skills

Knowledge & Skills

• Home health assessment and treatment planning
• Therapeutic exercise prescription and functional mobility training
• Safe transfer and gait training techniques
• Skilled documentation and charting (Epic preferred)
• Strong clinical reasoning and time management for solo visits
• Excellent communication and caregiver education skills
