Roles & Responsibilities

• Provide compassionate, hands-on nursing care to long term care residents
• Administer medications and treatments safely and according to policy
• Perform resident assessments and report changes in condition promptly
• Complete accurate documentation, including MDS-related entries when applicable
• Collaborate with the interdisciplinary care team to implement care plans
• Support residents with activities of daily living while preserving dignity
• Assist with admissions, transfers, and discharges as needed
• Promote a safe, comfortable environment and adhere to infection control practices

Qualifications

• Current, unrestricted Colorado LPN license required
• Long Term Care (LTC) experience required
• MDS experience preferred
• Strong clinical judgment and ability to prioritize resident needs
• Excellent communication and teamwork skills
• Ability to work the posted day shift hours reliably

Education

• Completion of an accredited Licensed Practical Nurse (LPN) program or vocational nursing diploma

Knowledge & Skills

• Knowledge of long term care nursing practices and resident-centered care
• Familiarity with MDS processes and documentation best practices
• Medication administration and basic wound care skills
• Accurate charting and clinical documentation abilities
• Strong interpersonal skills for working with residents, families, and staff
