Roles & Responsibilities

• Provide compassionate, evidence-based physical therapy to skilled nursing residents
• Perform thorough assessments and create individualized treatment plans
• Collaborate with the therapy team and nursing staff to optimize patient outcomes
• Document progress accurately and timely in accordance with facility policies
• Meet productivity expectations and contribute to a high-performing therapy program
• Engage residents and families to set meaningful functional goals

Qualifications

• PT or DPT (PT/DPT listed as core skills for this role)
• 1-3 years of experience working in a therapy setting (SNF experience a plus)
• Ability to meet facility productivity expectations (90% productivity requirement noted)
• Willingness to follow CRG policies and procedures
• Must be present in the building to receive guaranteed hours

Education

• DPT or equivalent Physical Therapy degree (DPT preferred)

Knowledge & Skills

• Skilled in physical therapy for skilled nursing facility (SNF) populations
• Strong teamwork and communication with interdisciplinary care teams
• Good documentation practices and familiarity with productivity metrics
• Comfort working in a fast-paced therapy environment with predictable expectations
• Professional presence and adherence to facility dress code (black scrubs)
