Roles & Responsibilities

• Assess patient mobility, balance, strength and functional needs using evidence-based evaluations
• Develop and implement individualized treatment plans to restore function and independence
• Provide therapeutic interventions including therapeutic exercise, gait training, and modalities as appropriate
• Document patient progress accurately and timely in the medical record, ensuring continuity of care
• Collaborate with nurses, physicians and the rehab team to coordinate comprehensive patient-centered care
• Educate patients and families on home exercise programs, safety strategies and fall prevention
• Participate in care conferences and contribute to quality improvement initiatives

Qualifications

• Current, unrestricted Physical Therapist license in California (required)
• Graduation from an accredited physical therapy program (DPT preferred)
• Current BLS/CPR certification
• Experience in skilled nursing, inpatient rehab, or post-acute settings is highly valued
• Strong documentation skills and comfort with electronic medical records
• Excellent communication skills and a collaborative bedside manner

Education

• Doctor of Physical Therapy (DPT) preferred or Bachelor's/Master's in Physical Therapy from an accredited program

Knowledge & Skills

• Knowledge of rehabilitation principles, therapeutic exercise and functional mobility training
• Skill in assessment techniques (gait, balance, transfers, ADL independence)
• Familiarity with mobility aids, orthotics and durable medical equipment recommendations
• Ability to create measurable, goal-oriented treatment plans
• Strong clinical reasoning and outcome-driven treatment approach
• Comfort working within an interdisciplinary team and mentoring support staff
