Roles & Responsibilities

• Assess residents’ functional abilities and design individualized occupational therapy plans that help them regain independence
• Deliver evidence-based OT interventions focused on ADLs, mobility, cognition, and adaptive equipment training
• Monitor, document, and communicate resident progress clearly and timely in the medical record
• Collaborate with nurses, PT, SLP, social services, and families as part of the interdisciplinary care team
• Train and mentor CNAs and nursing staff on therapeutic carryover and safe transfers
• Ensure compliance with facility policies, state regulations, and best-practice standards
• Discharge planning and equipment recommendations to support safe transitions

Qualifications

• Licensed Occupational Therapist (OTR/OT) in Texas, or ability to obtain Texas license prior to start
• Degree in Occupational Therapy (entry-level OTD, MSOT, or BS/entry-level as applicable)
• Current CPR/BLS certification (or willingness to obtain prior to start)
• Experience in skilled nursing/rehab settings preferred; new grads with strong clinical skills considered
• Reliable, professional communication skills and ability to work well in a team
• Ability to pass background checks and required health screenings

Education

• Bachelor’s or Master’s degree in Occupational Therapy (or equivalent professional degree)

Knowledge & Skills

• Strong knowledge of ADL retraining, adaptive equipment, and fall prevention strategies
• Skilled in therapeutic documentation and care planning in an EHR environment
• Proficient in assessment tools for functional mobility, cognition, and upper-extremity function
• Excellent clinical judgment and patient-centered problem solving
• Ability to educate patients, families, and staff with clarity and compassion
• Time management and caseload prioritization in a fast-paced rehab setting
