Roles & Responsibilities
- Provide compassionate, person-centered nursing care to residents at Bloomfield Nursing and Rehab
- Administer medications and treatments accurately and safely according to facility policy
- Perform thorough assessments and document changes in condition in the medical record
- Collaborate with RNs, therapists, and support staff to implement individualized care plans
- Support activities of daily living (ADLs) and promote resident independence and dignity
- Monitor and report clinical changes promptly to ensure timely interventions
- Maintain infection control practices and adhere to safety standards
Qualifications
- Valid LPN license in New Mexico (required)
- Current Basic Life Support (BLS) certification or willingness to obtain upon hire
- Previous experience in long-term care, skilled nursing, or rehab settings preferred
- Strong communication skills and ability to work effectively with interdisciplinary teams
- Ability to work overnight shifts (Tuesday-Thursday) and maintain attention to detail during extended hours
Education
- Completion of an accredited Practical Nursing (LPN) program or equivalent
Knowledge & Skills
- Medication administration and documentation best practices
- Resident assessments and recognizing changes in clinical condition
- Wound care basics and pressure injury prevention
- Charting and comfort using electronic medical records
- Safe patient handling, transfer techniques, and fall prevention
- Excellent interpersonal skills for working with residents and families
