Roles & Responsibilities
- Assess patients’ functional abilities and occupational needs in a skilled nursing setting
- Develop individualized treatment plans focused on meaningful daily activities
- Implement therapeutic interventions to improve independence and safety
- Collaborate with nursing and interdisciplinary teams to coordinate care
- Document progress and outcomes clearly and timely in the patient record
- Educate patients and families on strategies for daily living and safe transitions
- Support safe discharge planning with practical, patient-centered recommendations
Qualifications
- Any experience level welcome — including new grads
- Degree in Occupational Therapy (or completion of an accredited OT program)
- Licensed or eligible for Occupational Therapy practice in Illinois (facility will verify licensure requirements)
- Comfort working in a Skilled Nursing Facility (SNF) practice setting
- Strong clinical reasoning, documentation, and communication skills
Education
- Degree from an accredited Occupational Therapy program (per facility requirements); new grads accepted
Knowledge & Skills
- Skilled in occupational therapy assessments and intervention planning
- Experience or knowledge of SNF workflows and regulatory documentation
- Ability to collaborate with interdisciplinary teams
- Proficient documentation and clinical outcome tracking
- Patient- and family-centered teaching and discharge planning
