Roles & Responsibilities

• Deliver compassionate bedside care as a Licensed Practical Nurse, keeping patient safety front and center
• Administer medications and treatments accurately and document all care thoroughly
• Perform routine assessments and report any changes in patient condition to the care team
• Assist with wound care, IV support per facility policy, and other skilled nursing tasks
• Collaborate with RNs, therapists, and physicians to create and follow patient care plans
• Educate patients and families about daily care, medications, and discharge instructions
• Maintain a clean, safe environment and follow infection control best practices
• Contribute to a positive, team-oriented culture that improves patient outcomes

Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing (PA license preferred)
• Basic Life Support (BLS) / CPR certification (or similar) required or must obtain
• Comfortable working in a skilled nursing or long-term care environment
• Strong communication skills and a patient-first attitude
• Ability to pass facility onboarding, background check, and health requirements

Education

• Completion of an accredited LPN / practical nursing program
• Vocational diploma or certificate in Practical Nursing

Knowledge & Skills

• Medication administration and documentation
• Patient assessment and vital sign monitoring
• Wound care and basic clinical procedures
• Familiarity with electronic medical records and charting
• Infection control and safety protocols
• Time management and prioritization during long shifts
• Team collaboration and clear communication with multidisciplinary staff
