Roles & Responsibilities

• Provide compassionate direct resident care with dignity and respect
• Assist with activities of daily living including bathing, dressing, grooming and mobility
• Monitor and document residents’ conditions and report changes to nursing staff
• Support safe and comfortable mealtimes and follow dietary instructions
• Help create a warm, home-like environment that promotes resident well-being
• Collaborate with the care team to follow care plans and meet individualized needs

Qualifications

• Licensed Nursing Assistant (LNA) required; Travel LNA (T-LNA) accepted
• Active/clear license or eligible for state licensing in Vermont
• Availability to work weekends as required by the assignment
• Comfortable working in a long-term care or skilled nursing environment
• Strong communication and teamwork skills; dependable and resident-focused

Knowledge & Skills

• Basic patient care and ADL assistance
• Vital signs monitoring and documentation
• Infection control and safety procedures
• Solid interpersonal skills for working with residents and families
• Ability to follow care plans and communicate observations clearly
