Roles & Responsibilities

• Assess patients to identify functional strengths and areas to improve, creating person-centered OT plans
• Develop and implement therapeutic interventions to restore independence in daily living skills
• Provide skilled treatments in a Skilled Nursing Facility setting that directly impact patient quality of life
• Collaborate with nurses, PTs, SLPs and the interdisciplinary team to coordinate care and meet goals
• Document progress, discharge plans and treatment notes accurately and timely
• Educate patients and families on strategies, adaptive equipment and home safety to support long-term success
• Advocate for patient needs and help transition patients safely to next levels of care

Qualifications

• Licensed Occupational Therapist (OT) in Arizona or eligible for Arizona licensure
• OTR/L preferred; new graduates with current license or pending licensure are encouraged to apply
• Experience in Skilled Nursing Facility settings is a plus but not required
• Current Basic Life Support (BLS) certification or willingness to obtain
• Strong clinical reasoning, documentation skills, and a team-oriented mindset

Education

• Entry-level and experienced Occupational Therapists welcome
• Degree in Occupational Therapy (BS/BS/OT, MOT, MSOT or OTD) required for licensure

Knowledge & Skills

• Skilled in assessments and developing individualized treatment plans
• Knowledge of ADLs, adaptive equipment, and fall prevention strategies
• Comfortable working in a SNF practice setting and collaborating with interdisciplinary teams
• Strong documentation and time-management skills
• Patient-centered communication and family education abilities
