Roles & Responsibilities
- Assess patients to identify functional strengths and areas to improve, creating person-centered OT plans
- Develop and implement therapeutic interventions to restore independence in daily living skills
- Provide skilled treatments in a Skilled Nursing Facility setting that directly impact patient quality of life
- Collaborate with nurses, PTs, SLPs and the interdisciplinary team to coordinate care and meet goals
- Document progress, discharge plans and treatment notes accurately and timely
- Educate patients and families on strategies, adaptive equipment and home safety to support long-term success
- Advocate for patient needs and help transition patients safely to next levels of care
Qualifications
- Licensed Occupational Therapist (OT) in Arizona or eligible for Arizona licensure
- OTR/L preferred; new graduates with current license or pending licensure are encouraged to apply
- Experience in Skilled Nursing Facility settings is a plus but not required
- Current Basic Life Support (BLS) certification or willingness to obtain
- Strong clinical reasoning, documentation skills, and a team-oriented mindset
Education
- Entry-level and experienced Occupational Therapists welcome
- Degree in Occupational Therapy (BS/BS/OT, MOT, MSOT or OTD) required for licensure
Knowledge & Skills
- Skilled in assessments and developing individualized treatment plans
- Knowledge of ADLs, adaptive equipment, and fall prevention strategies
- Comfortable working in a SNF practice setting and collaborating with interdisciplinary teams
- Strong documentation and time-management skills
- Patient-centered communication and family education abilities
