Roles & Responsibilities

• Conduct comprehensive biopsychosocial assessments to understand patient needs and strengths
• Develop and document individualized care plans and goals in EPIC that guide patient care
• Provide compassionate one-on-one counseling and facilitate group counseling sessions for older adults
• Coordinate care with internal and external team members, referring to appropriate disciplines and resources
• Lead discharge planning and care coordination to ensure safe transitions and continuity
• Perform cognitive screening using tools like the Geriatric Depression Scale and MMSE
• Deliver crisis intervention and short-term therapy when patients present acute needs
• Document clinical observations, care delivery, and progress clearly and timely in EPIC
• Participate in care conferences and interdisciplinary meetings to improve patient outcomes
• Educate patients and caregivers to support long-range planning and daily care needs

Qualifications

• Active LMSW with a Washington state license required at time of signature
• BLS certification required
• Minimum 1 year of experience working with geriatrics
• EPIC EMR experience required
• Master of Social Work (MSW) preferred
• Comfort with both individual (1:1) patient care and group counseling
• Reliable transportation and no additional scheduling restrictions
• Willingness to consider permanent placement at the facility after the assignment

Education

• Licensed Master Social Worker (LMSW) — Washington state license required
• Master of Social Work (MSW) preferred

Knowledge & Skills

• Biopsychosocial assessment and development of individualized care plans
• Strong documentation skills and familiarity with EPIC charting
• Care coordination, discharge planning, and case management
• Experience with geriatric mental health, dementia care, and complex medical/social needs
• Use of screening tools such as the Geriatric Depression Scale (GDS) and MMSE
• Crisis intervention, short-term therapy techniques, and substance abuse treatment knowledge
• Ability to collaborate with interdisciplinary teams and refer to community resources
• Excellent communication and patient/caregiver education skills
