Roles & Responsibilities
- Provide compassionate direct patient care and assist with activities of daily living (bathing, dressing, feeding)
- Perform vital signs and routine clinical observations, reporting changes promptly to the nursing team
- Support patient mobility and safe transfers while following proper body mechanics and safety protocols
- Maintain clear, accurate documentation of care and patient status in the chart/EMR
- Collaborate with nurses, therapists, and multidisciplinary team members to support care plans
- Uphold infection control standards and facility policies to ensure a safe environment for patients and staff
Qualifications
- Current Certified Nursing Assistant (CNA) or T-CNA certification (required)
- Active BLS/CPR certification preferred or able to obtain prior to start
- Ability to pass background check and drug screen as required by facility
- Clinical experience in long-term care or similar setting preferred
- Strong communication skills and a patient-first attitude
- Physical ability to perform patient transfers, assist with mobility, and handle typical CNA duties
Education
- High school diploma or equivalent and successful completion of a state-approved CNA program
Knowledge & Skills
- Hands-on knowledge of ADLs, toileting, feeding assistance and basic patient hygiene
- Ability to take and record vital signs accurately
- Familiarity with infection prevention and control practices
- Comfortable with basic charting and communicating observations to the RN
- Team-oriented mindset with good time management and prioritization
- Patient safety focus, including fall prevention and safe transfer techniques
