Roles & Responsibilities

• Provide compassionate direct patient care and assist with activities of daily living (bathing, dressing, feeding)
• Perform vital signs and routine clinical observations, reporting changes promptly to the nursing team
• Support patient mobility and safe transfers while following proper body mechanics and safety protocols
• Maintain clear, accurate documentation of care and patient status in the chart/EMR
• Collaborate with nurses, therapists, and multidisciplinary team members to support care plans
• Uphold infection control standards and facility policies to ensure a safe environment for patients and staff

Qualifications

• Current Certified Nursing Assistant (CNA) or T-CNA certification (required)
• Active BLS/CPR certification preferred or able to obtain prior to start
• Ability to pass background check and drug screen as required by facility
• Clinical experience in long-term care or similar setting preferred
• Strong communication skills and a patient-first attitude
• Physical ability to perform patient transfers, assist with mobility, and handle typical CNA duties

Education

• High school diploma or equivalent and successful completion of a state-approved CNA program

Knowledge & Skills

• Hands-on knowledge of ADLs, toileting, feeding assistance and basic patient hygiene
• Ability to take and record vital signs accurately
• Familiarity with infection prevention and control practices
• Comfortable with basic charting and communicating observations to the RN
• Team-oriented mindset with good time management and prioritization
• Patient safety focus, including fall prevention and safe transfer techniques
