Roles & Responsibilities

• Provide compassionate assistance with activities of daily living (bathing, dressing, grooming) to promote patient dignity
• Take and record vital signs accurately and communicate changes to the nursing team
• Assist with safe transfers and mobility, using proper body mechanics and lift equipment when needed
• Support mealtime assistance and monitor dietary needs to help patients maintain nutrition
• Document care thoroughly and timely in the patient record to ensure continuity of care
• Observe and report changes in patient condition to licensed nursing staff to support positive outcomes
• Work collaboratively with the interdisciplinary team to create a comfortable, safe environment for residents

Qualifications

• Current CNA certification (required) — ability to work as a Certified Nursing Assistant in Pennsylvania
• CPR/BLS certification preferred or willingness to obtain upon hire
• Comfortable assisting with ADLs and basic patient care tasks
• Positive attitude, strong communication skills, and a team-first mindset
• Background check and required immunizations/health screenings completed per facility policy

Education

• High school diploma or GED and completion of a state-approved CNA training program

Knowledge & Skills

• Solid understanding of infection control and patient safety practices
• Proficiency taking and recording vital signs and weight
• Comfortable with resident handling, transfers, and fall prevention techniques
• Good documentation skills and familiarity with electronic or paper charting
• Strong interpersonal skills for working with residents, families, and the care team
