Roles & Responsibilities

• Provide compassionate, person-centered bedside care to residents and patients
• Assist with activities of daily living including bathing, dressing, and mobility
• Monitor and report changes in patient condition to the nursing team promptly
• Document care accurately and maintain clear communication with care partners
• Support safe transfers and fall-prevention measures to keep patients secure
• Collaborate with nurses and therapists to follow individualized care plans
• Promote dignity and independence while delivering routine nursing tasks

Qualifications

• Active CNA certification (Certified Nursing Assistant) — required
• Current state-required licensure and credentials for Pennsylvania CNAs
• Comfortable working in long-term care or rehabilitation settings
• Strong communication skills and ability to work as part of a team
• Dependable, compassionate, and patient-focused attitude

Education

• High school diploma or GED preferred; completion of an accredited CNA program required

Knowledge & Skills

• Hands-on patient care and safe transfer techniques
• Basic vital signs and observing/reporting clinical changes
• Infection control practices and proper PPE use
• Clear documentation and charting skills
• Effective communication with patients, families and clinical staff
• Time management and prioritization during busy shifts
