Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a long-term care/rehab setting
• Administer medications and treatments safely and accurately
• Document resident status, interventions and responses thoroughly and timely
• Monitor vital signs and report significant changes to the care team
• Assist residents with activities of daily living including bathing, dressing and mobility
• Collaborate with nurses, therapists and physicians to implement and update care plans
• Educate residents and families on care plans, medications and safe practices
• Maintain infection control standards and a safe environment for residents

Qualifications

• Active LPN license (Licensed Practical Nurse); Massachusetts licensure preferred
• Current BLS/CPR certification
• Previous experience in long-term care, rehab or skilled nursing preferred
• Valid immunizations and TB screening as required by facility policy
• Strong communication skills and a team-first attitude

Education

• Practical Nursing diploma or certificate (completion of an accredited LPN program)

Knowledge & Skills

• Medication administration and accurate documentation
• Basic wound care and assessment
• Proficient in taking and interpreting vital signs
• Comfortable using electronic health records for charting
• Patient-centered care, compassion and time management in busy clinical settings
