Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a long-term care/rehab setting
- Administer medications and treatments safely and accurately
- Document resident status, interventions and responses thoroughly and timely
- Monitor vital signs and report significant changes to the care team
- Assist residents with activities of daily living including bathing, dressing and mobility
- Collaborate with nurses, therapists and physicians to implement and update care plans
- Educate residents and families on care plans, medications and safe practices
- Maintain infection control standards and a safe environment for residents
Qualifications
- Active LPN license (Licensed Practical Nurse); Massachusetts licensure preferred
- Current BLS/CPR certification
- Previous experience in long-term care, rehab or skilled nursing preferred
- Valid immunizations and TB screening as required by facility policy
- Strong communication skills and a team-first attitude
Education
- Practical Nursing diploma or certificate (completion of an accredited LPN program)
Knowledge & Skills
- Medication administration and accurate documentation
- Basic wound care and assessment
- Proficient in taking and interpreting vital signs
- Comfortable using electronic health records for charting
- Patient-centered care, compassion and time management in busy clinical settings
