Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a skilled nursing and rehab setting
- Administer medications and treatments accurately and document in the medical record
- Monitor patient status, report changes promptly, and escalate concerns to the care team
- Collaborate with RNs, therapists, and support staff to create and carry out individualized care plans
- Support admissions, discharges, and transitions with attention to safety and continuity of care
- Educate patients and families about care plans, safety, and daily routines with empathy and clarity
- Maintain infection prevention practices and ensure a safe, clean environment for patients and staff
Qualifications
- Licensed Practical Nurse (LPN) credential required
- Active state LPN license or ability to obtain state licensure promptly
- Basic Life Support (BLS/CPR) certification preferred or willing to obtain
- Previous experience in long-term care, rehab, or skilled nursing is a plus
- Strong communication skills and ability to work collaboratively with multidisciplinary teams
- Reliable, professional, and committed to patient-focused care
Education
- Completion of an accredited Practical Nursing program
- High school diploma or equivalent
Knowledge & Skills
- Skilled in medication administration and documentation accuracy
- Comfortable performing routine wound care and monitoring clinical changes
- Familiarity with electronic medical records and charting best practices
- Good assessment skills for vital signs, ADLs, and basic nursing observations
- Infection control, safety procedures, and resident rights awareness
- Strong interpersonal skills for patient and family education and support
