Roles & Responsibilities

• Provide skilled speech-language pathology assessment and treatment for SNF residents
• Develop and implement individualized treatment plans that improve communication and swallowing outcomes
• Collaborate with an interdisciplinary therapy team to coordinate patient-centered care
• Document clinical progress and maintain timely, accurate records per facility and CRG policy
• Meet productivity expectations and contribute to a positive, efficient therapy environment
• Engage residents and families with compassionate education and supportive strategies

Qualifications

• 1-3 years of SLP experience (as listed in the job posting)
• Experience working in skilled nursing facility (SNF) settings (core skill noted)
• Ability to be present onsite to receive guaranteed hours and meet facility requirements
• Willingness to follow CRG policies & procedures and meet productivity expectations (90%)
• Professional demeanor consistent with facility dress code (black scrubs) and teamwork values

Knowledge & Skills

• Skilled in assessment and treatment of communication and swallowing disorders in older adults
• Familiarity with SNF workflows and interdisciplinary collaboration
• Strong clinical documentation and time-management skills
• Ability to meet productivity standards while maintaining high-quality care
• Comfort working within established facility policies and protocols
