Roles & Responsibilities

• Conduct comprehensive speech-language evaluations to identify patient needs
• Provide daily therapeutic treatments tailored to individual goals
• Manage safe and timely discharges with clear documentation and care plans
• Collaborate closely with the therapy team to coordinate patient-centered care
• Follow Continuum Rehabilitation Group (CRG) policies and facility procedures
• Maintain productivity standards and accurate clinical notes
• Present onsite to ensure continuity of care and to receive guaranteed hours

Qualifications

• Active Speech-Language Pathologist credentials (state license required or ability to obtain)
• 1–3 years of clinical SLP experience preferred
• Prior skilled nursing facility (SNF) experience strongly preferred
• Comfortable meeting productivity expectations (facility notes a 90% requirement)
• Professional, reliable presence in the building as required by the contract

Knowledge & Skills

• Skilled in speech-language evaluations and treatment planning
• Experience with daily therapy sessions and discharge planning in SNF settings
• Strong documentation skills and ability to meet productivity targets
• Excellent communication and teamwork within multidisciplinary rehab teams
• Understanding of facility policies, infection control, and on-site protocols
