Social Worker
Location: Prescott, AZ

Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (five 8-hour shifts; guaranteed 40 hours/week). May occasionally include a weekend shift approximately every 3-4 weeks. Occasional Emergency Department coverage 10:00 AM - 6:30 PM. Day shift.

Candidate Pay Rate: $50 per hour

Roles & Responsibilities
- Conduct comprehensive social work and mental health assessments for inpatient and emergency department patients
- Manage a caseload of approximately 12-16 consults per day with thoughtful prioritization
- Develop and coordinate individualized discharge plans that connect patients to community resources
- Complete Title 36 petitions and perform DCS and APS reporting when indicated
- Provide SDOH consults and maternal-child assessments, addressing homelessness and substance abuse needs
- Document all interventions and case notes accurately and timely in Cerner EMR
- Collaborate with interdisciplinary teams across inpatient units, ICU, ED, and Family Birthing Center
Qualifications
- LMSW or LCSW required
- Minimum 1 year of relevant clinical experience
- Proficiency with Cerner EMR for documentation and charting
- Demonstrated experience in discharge planning, crisis intervention, and mental health assessments
- Comfort providing coverage in the Emergency Department and across inpatient settings
- Ability to manage a daily caseload of ~12-16 consults with strong organizational skills
- Must meet radius rule: within 50 miles and not been full-time, part-time, or PRN with CommonSpirit (including CHI or Dignity) within the past year
- BLS is not required
Knowledge & Skills
- Strong social work assessment and clinical interviewing skills
- Knowledge of Title 36 procedures and mandated reporting (DCS, APS)
- Experience addressing social determinants of health and connecting community resources
- Familiarity with maternal-child assessments and substance abuse interventions
- Excellent documentation and caseload management practices
- Effective interdisciplinary communication and care coordination
- Ability to work in fast-paced acute care, ED, ICU, and birthing center environments
