Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Provide compassionate care coordination for patients in need of mental health support
Conduct thorough assessments and facilitate 5150 involuntary evaluations when appropriate
Collaborate with interdisciplinary teams to develop individualized care plans
Support patients and families through crisis intervention and resource navigation
Document clinical encounters accurately and maintain compliance with all regulations

Qualifications

Active California LCSW license required
Current BLS certification
5150 certification in California required
Prior experience working at a Dignity Health facility
Strong assessment and crisis intervention skills

Knowledge & Skills

Expertise in involuntary psychiatric holds and 5150 processes
Ability to work collaboratively within multidisciplinary teams
Excellent communication and documentation skills
Understanding of care coordination best practices
