Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver personalized physical therapy services directly in patients' homes
Help patients recover from surgeries, falls, and other conditions with compassion and expertise
Collaborate with Home Health and Hospice teams to support holistic patient care
Document care accurately using HomeCare/HomeBase EMR
Travel within a 60-mile radius to bring expert therapy to those who need it most

Qualifications

Current North Dakota Physical Therapy License
BLS certification required
At least 1 year of previous home health experience
Valid driver's license and auto insurance
Strong PT-related clinical skills

Knowledge & Skills

Home health physical therapy expertise
Comfortable working independently in patients' homes
Familiarity with HomeCare/HomeBase EMR
Excellent time management and organizational abilities
Compassionate approach to patient-centered care
