Roles & Responsibilities
- Provide compassionate care coordination for patients in need of mental health support
- Complete involuntary 72-hour psychiatric evaluations when individuals may be a danger to themselves or others
- Collaborate with interdisciplinary teams to ensure seamless care transitions
- Maintain accurate documentation while wearing provided Care Coordination lab coats
Qualifications
- Active California LCSW license required
- Current BLS certification
- 5150 certification in California
- Prior experience with Dignity Health facilities
- Two supervisory references needed at time of application
Knowledge & Skills
- Understanding of involuntary psychiatric holds and mental health crisis intervention
- Strong communication and care coordination abilities
- Professional business casual presentation
