Location: Red Bluff, California

Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Deliver personalized physical therapy care to patients in the comfort of their homes
- Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
- Help patients recover mobility and independence after joint replacements and other procedures
- Support geriatric patients managing multiple chronic conditions like CHF and COPD
- Document visits accurately using HomeCare HomeBase EMR on your provided tablet
- Collaborate with the intake RN and manager to coordinate seamless patient care
Qualifications
- Active Physical Therapy license in California
- BLS certification required
- Home Health experience preferred but will consider candidates with less than 2 years if they have Home Health background
- Strong assessment and documentation skills
Knowledge & Skills
- Physical Therapy evaluation and treatment planning
- Home Health documentation and compliance
- Orthopedic and geriatric patient care
- Comfortable working independently in home settings
- Familiarity with HomeCare HomeBase EMR a plus
