Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across Yolo County
Assess and evaluate each patient's therapeutic needs, mobility, gait, and range of motion
Develop and update individualized plans of care in collaboration with physicians
Educate patients and families on therapeutic exercises and safe use of equipment
Monitor progress and communicate changes in condition to the care team
Coordinate with home health aides and other professionals to support rehabilitation goals
Document clinical findings and progress summaries to ensure continuity of care

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Ability to work independently while thriving as part of a collaborative team

Knowledge & Skills

Physical Therapy assessment and treatment planning
Home environment evaluation and safety recommendations
Therapeutic exercise instruction and gait training
Range of motion and muscle strength assessment
Patient and caregiver education techniques
Clear documentation and progress reporting
