Roles & Responsibilities
- Deliver personalized physical therapy to patients across four rural counties
- Conduct OASIS assessments and Start of Care evaluations
- Help patients recover from falls, fractures, and other common conditions
- Manage a manageable caseload of 4-6 visits per day
- Document visits and track time using a tablet for seamless workflow
Qualifications
- Current Physical Therapist license in Indiana
- Experience with OASIS assessments and Start of Care documentation
- Comfortable working independently in rural settings
- Strong communication and time-management skills
