Hours per Week: 40

Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Coordinate seamless patient care transitions that make a real difference
Master complex discharge planning to ensure safe patient journeys home
Leverage Meditech and Midas systems to optimize case management workflows
Collaborate with RN and SW teams to deliver outstanding outcomes
Advocate for patients with a 25:1 ratio that keeps your impact front and center

Qualifications

Active RN license in good standing
At least 1 year of case management experience
Proficiency in Midas and Meditech systems required
Florida RN license or ability to obtain quickly for travel pros

Education

RN degree from accredited nursing program

Knowledge & Skills

Expertise in Meditech and Midas EMR systems
Strong skills in complex discharge planning
RN case management core competencies
Team collaboration with social workers and staff
