Location: Carmichael, California

Contract Type: Contract

Shift Type: 8-hour shifts, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, rotating weekends every other weekend, rotating holidays with core staff

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Develop patient daily lists and review patient charts
Assess patient needs and initiate goals of care discussions
Educate patients and families on disease processes and advance directives (POLSTs)
Lead family meetings and provide end-of-life support
Attend daily rounds and collaborate with interdisciplinary team
Coordinate care for 10-15 consults per day with a focus on palliative care

Qualifications

2-5 years of recent experience in palliative and/or hospice setting
ICU or oncology experience is a plus
Strong acute care, clinical thinking, and communication skills
Flexibility and strong organizational skills a must
Desire to serve patients, families, and team members
