Shift Type: Monday-Friday, 8:00 AM - 5:00 PM; rotating weekends and holidays

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Perform start of care, recertifications, and re-evaluations for home health patients
Provide skilled physical therapy visits to patients with CHF, ortho conditions, and pneumonia
Collaborate with clinical managers and educators for guidance and support
Document patient progress and adjust treatment plans as needed
Cover assigned territory in the Louisville area

Qualifications

Active Physical Therapist license in Kentucky
BLS certification required
New grads welcome to apply; experienced therapists preferred
Home health experience is a plus but not required
Must be comfortable with rotating weekend and holiday schedules

Knowledge & Skills

Skilled in evaluating and treating a variety of diagnoses including CHF, ortho, and pneumonia
Proficient in documentation and care planning
Strong communication and teamwork skills
Ability to work independently in a home health setting
