Roles & Responsibilities

• Assess patients thoroughly and create individualized, evidence-based treatment plans
• Deliver skilled physical therapy interventions to help residents regain function and independence
• Document patient progress and plan updates clearly and on time
• Collaborate with nursing and interdisciplinary teams to coordinate excellent patient care
• Coach patients and families on home programs and safety to support long-term outcomes
• Maintain productivity standards and follow facility policies and procedures to ensure consistent care

Qualifications

• Licensed Physical Therapist (PT) in New Mexico or eligible for licensure
• Doctor of Physical Therapy (DPT) preferred; PT degree required
• 1 to 3 years of experience working in a skilled nursing facility or similar setting preferred
• Comfortable working on-site and following facility dress code (black scrubs)
• Ability to meet productivity expectations and follow employer policies

Education

• Doctor of Physical Therapy (DPT) preferred; graduation from an accredited PT program and active PT licensure

Knowledge & Skills

• Skilled in patient assessment, treatment planning and therapeutic exercise
• Experience with skilled nursing facility (SNF) workflows and documentation
• Strong communication and teamwork skills to collaborate across disciplines
• Good time management and productivity focus to meet clinical targets
• Ability to adapt care plans to diverse patient needs and recovery goals
