Roles & Responsibilities
- Deliver personalized physical therapy services to patients recovering from surgeries and falls in their homes
- Provide compassionate care to both home health and hospice patients across the Valley City region
- Collaborate with interdisciplinary teams to support patient recovery goals
- Document patient progress using HomeCare/HomeBase EMR system
- Travel within a 60-mile radius to deliver high-quality therapy services where patients need it most
Qualifications
- Current North Dakota Physical Therapy License required
- BLS certification needed
- At least 1 year of previous home health experience
- Valid driver's license and auto insurance required
- HomeCare/HomeBase EMR experience preferred
Knowledge & Skills
- Physical Therapy expertise in home health settings
- Comfort working independently with minimal clinical oversight
- Strong communication and documentation abilities
- Familiarity with common diagnoses including post-surgical recovery and fall prevention
