Social Worker

Location: Prescott, Arizona

Start Date: 06-29-2026

End Date: 09-28-2026

Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 4:30 PM, with possible weekend rotation every 3-4 weeks

Hours per Week: 40

Candidate Pay Rate: $50 per hour

Roles & Responsibilities

Complete comprehensive social work assessments for patients across inpatient, emergency, ICU, and family birthing units
Conduct mental health assessments and support patients in crisis
Coordinate discharge planning and connect patients with vital community resources
Assist with Title 36 petitions and address complex social determinants of health
Support families facing homelessness, substance abuse challenges, and maternal-child concerns
Collaborate with DCS and APS as needed to ensure patient safety

Qualifications

Active LMSW or LCSW license in Arizona
Experience with Cerner EMR preferred
Strong assessment and discharge planning skills
Compassionate approach to diverse patient populations

Knowledge & Skills

Social work assessment expertise
Mental health evaluation skills
Discharge planning proficiency
Knowledge of Title 36 processes
Understanding of social determinants of health
Familiarity with DCS and APS reporting requirements
