Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Provide exceptional physical therapy care to patients in their homes throughout Tehama County
- Conduct Start of Care visits especially with orthopedic patients when nursing is not ordered
- Help patients recover from joint replacements and manage chronic conditions like CHF and COPD
- Complete approximately 5 meaningful patient visits each day using your clinical expertise
- Document care efficiently using the HCHB HomeCare HomeBase EMR system on your provided tablet
Qualifications
- Current Physical Therapy license in California
- BLS certification required
- Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
- Strong clinical skills for treating orthopedic and geriatric patients with multiple comorbidities
Knowledge & Skills
- Physical Therapy expertise in home health settings
- Comfortable working independently in patient homes
- Familiarity with HCHB HomeCare HomeBase EMR system
- Excellent time management for managing daily visit schedules
- Compassionate approach to geriatric care
