Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM (Day shift)

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct initial start-of-care visits for orthopedic and geriatric patients in their homes
Develop and implement individualized treatment plans to maximize patient independence
Manage a daily caseload of 5 visits, covering Tehama County and surrounding areas
Collaborate with nursing and therapy team members via HCHB EMR
Document patient progress and communicate changes to the supervising clinician

Qualifications

Active Physical Therapist license in California
Home Health experience required (new travelers welcome if they have this experience)
BLS certification from the American Heart Association
Experience with HCHB (HomeCare HomeBase) EMR preferred

Knowledge & Skills

Expertise in orthopedic rehabilitation and geriatric care
Proficient in OASIS documentation and SOC visits
Strong time management and organizational skills
Ability to work independently in a home health setting
