Roles & Responsibilities
- Deliver personalized physical therapy care to patients in their homes
- Create customized treatment plans that help patients regain strength and mobility
- Collaborate with physicians and care teams to ensure the best outcomes
- Educate patients and families on exercises and recovery techniques
- Document progress and adjust care plans as needed
Qualifications
- Active California Physical Therapist license
- BLS certification
- Home health experience preferred
- Strong communication and time-management skills
Knowledge & Skills
- Home health physical therapy expertise
- Patient assessment and care planning
- Ability to work independently in various home settings
- Compassionate bedside manner
