Roles & Responsibilities

• Provide high-quality home health physical therapy evaluations and individualized treatment plans
• Deliver hands-on and evidence-based interventions to improve mobility, safety, and function
• Collaborate with nurses, case managers, and family members to coordinate patient-centered care
• Document progress clearly and timely in the patient record (Epic experience preferred)
• Educate patients and caregivers on home exercise programs and safe home modifications
• Perform required competency testing and participate in quality improvement initiatives

Qualifications

• Licensed Physical Therapist (current and active) — ability to practice in the assignment state
• BLS (Basic Life Support) certification required
• Home health experience preferred; strong clinical assessment and documentation skills
• Comfortable with community-based care and autonomous decision-making in patient homes
• Willingness to complete Trinity specialty competency testing and required health screens
• Reliable transportation and ability to manage a patient caseload across the service area

Education

• Graduate of an accredited Physical Therapy program (DPT preferred or equivalent)

Knowledge & Skills

• Expertise in geriatric and adult mobility assessment
• Skilled in creating practical home exercise programs and functional training
• Solid documentation skills; Epic experience is preferred but not required
• Ability to triage and prioritize visits while maintaining clinical quality
• Strong communication and patient-family education skills
• Familiarity with home safety assessment and adaptive equipment recommendations
