Roles & Responsibilities

• Assess patients in their homes and develop individualized treatment plans that focus on functional goals
• Deliver skilled therapeutic interventions to improve mobility, safety, and independence
• Collaborate with nurses, social workers, and physicians to coordinate comprehensive home care
• Document patient evaluations, treatment plans, and progress thoroughly in the electronic health record (Epic preferred)
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility strategies
• Complete required Trinity specialty competency testing and skills checklists to ensure quality care
• Maintain licensure, certifications, and required health screenings to meet onboarding standards

Qualifications

• Active Physical Therapist license (state-appropriate) — required
• BLS certification — required
• Home health experience preferred; comfortable working independently in patient homes
• Epic experience preferred but not required
• Ability to complete Trinity Specialty Specific Competency Testing and Skills Checklist
• Must be able to complete required health screenings (drug screen, immunizations, fit test, etc.) and license/certificate verification

Knowledge & Skills

• Strong home health assessment and care-planning skills
• Skilled in therapeutic interventions for mobility, balance, and ADLs
• Comfortable with thorough, timely documentation and electronic charting (Epic preferred)
• Excellent patient and caregiver education skills
• Knowledge of infection control and safety practices in the home setting
• Good communication and interdisciplinary teamwork abilities
