Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 4:30 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct thorough Physical Therapy evaluations to understand each patient's unique needs
Develop personalized plans of care that guide patients on their road to recovery
Participate in patient rounds and care conferences to ensure seamless communication across the care team
Collaborate with the rehab team on patient progress, equipment needs, and discharge planning
Provide therapy across acute, swing bed, nursing home, and outpatient settings as needed

Qualifications

Active NC Physical Therapy license
Current CPR certification
At least 2 years of experience in a hospital or nursing home setting preferred
Strong communication skills and a collaborative spirit

Knowledge & Skills

Physical Therapy evaluations and plan of care development
Patient rounds and care conference participation
Cerner and NetHealth documentation systems
Ability to float between acute, swing, nursing home, and outpatient services
